What to bring to your first visit?
We work hard to ensure that your child receives prompt medical care in a comfortable, child-friendly office setting. To help expedite the first appointment. The legal parent or guardian must accompany the child to the first visit with a photo ID and the following items:
- Primary Insurance card(s)
- Referral forms (if required)
- List of all prescription and over-the-counter medications that your child is currently taking
- Co-payment (if required)
- Up to Date Immunization Record
- Any past medical and surgical information
What to Expect
On the day of your child's first visit to our office, we require you to arrive 20 minutes early. This allows extra time for finishing up any registration forms and ensures you have plenty of time to get acquainted with our staff and office. We look forward to meeting you and joining with you to help your child maintain optimal health.
Insurance and Payment Policy
Child Care Consultants participates in most managed healthcare insurance plans. Please see the list below for your reference. This list may change and may not be the most accurate at all times. Please contact us directly or call the member services phone number on your insurance card to verify the Child Care Consultants or one of our physicians are providers for your insurance.
Many plans require a co-payment or deductible at the time of your visit. To accommodate this we accept cash, check, and Visa, MasterCard, and Discover credit cards. It is the requirement of most insurance companies that a co-payment be made at the time of service. Your co-payment amount is usually posted on the front of your insurance card.

If you are covered by and HMO insurance plan please make sure you list us as the Primary Care Physician.
Newborns must be added to your insurance plan within 30 days of date of birth. If newborns are not added to your insurance payment in full is expected.
Self Pays:
All self pay patients are required to pay the visit and procedures in full at the time of the visit.
